2014 CPT® Code Update Form

2014 CPT® codes are available to be downloaded into your DuxWare program. Any fees and practice
created descriptions that you added in your existing codes will not be changed when updated. New
codes will automatically be added; however, deprecated codes will not be deleted as they have been
used in prior claims and may still be needed for use in claims. Deletions of deprecated codes must be
made manually by you.

2014 ICD-9 codes will not have a data update due to the impending release of ICD-10 due out in October
2014. Information about the partial freeze of ICD-9 codes can be found on the CMS website or at the
following web address http://www.cms.gov/Medicare/Coding/ICD10/ICD-9-CM-Coordination-and-
Maintenance-Committee-Meetings.html

Pricing for 2014 CPT® code sets is $ 25.00 per practice – To order your codes, please fill out this form
and send it along with your payment to Medical Practice Software. PO Box 1488, Slidell, LA 70459.

Or, you may fax it to Medical Practice Software at 985-646-1676. When we receive your fax, an invoice
will be sent to your office for payment. Your codes will be updated automatically when payment is
received.
