===CPT Code Updates for 2019=== If your practice wishes to update your system with the 2019 CPT codes, this is optional as it is every year and falls under a new license agreement we have in place with the American Medical Association (AMA). Licensing fees for CPT codes are now calculated at $ 50.00 per user of CPT codes. According to the AMA, a user of the CPT files is one who needs CPT codes in the system in order to do any part of their job. Most likely all billing staff should be counted as a user under this license agreement. The 2019 CPT codes are available now for delivery. If you choose to purchase them, they will be automatically uploaded to your system upon receipt of this signed form. To order your 2019 CPT codes, please fill in the blanks below and fax a signed copy of this page to our office at (985) 646-1676. Per your order, your practice will see the user license fee of $ 50.00 per user. In addition, you will also be provided, via the email address* you provide in your order below, a receipt as notification that the 2019 CPT codes have been uploaded to your system. Make sure that the email address you supply is a valid email address, as it will be used for future communications from us when we need to provide additional information to you about your system.
